Office Policy

Thank you for selecting Dr. Craig Johnson for your endodontic care. We are committed to serving this community and providing you with the highest quality dental care possible. In order to achieve out commitment, it is necessary to implement the following financial policy.

In effort to keep clear insurance coverage versus patient portions, dental insurance is a contract between the insurance company and the patient; we are not a party to that contract. Therefore, it is the patient who is responsible for the bill, regardless of insurance coverage.
As a courtesy, our office will submit charges for services to your insurance company. We do however require the patient to pay for their estimated portion of the service not covered by insurance at the time of the service. If you’re insurance company pays in addition to what you have already paid on your total bill, a refund will be sent to you promptly.
If you have dental insurance, please bring all of the necessary information with you to your dental appointment. We can not bill your insurance unless you have provided us with all insurance information.
Patients without dental benefits: full payment is required at the time of service. Acceptable forms for payment include; cash, check Visa or MasterCard.
Missed appointments: Root canal treatment is a surgical procedure. The time reserved for you is one hour to one and a half hours. Once an appointment has been made, this time has been reserved exclusively for you with appropriate staff to serve your dental needs. Canceling without notice makes it difficult for us to meet the needs of other patients.
